Four Levels of Support
Home Care Packages are available at four levels, based on your individual care needs.
Level 1
Basic Care Needs
For people with basic care needs who need some assistance to remain living independently at home.
Level 2
Low Level Care Needs
For people with low level care needs who require assistance with some aspects of daily living.
Level 3
Intermediate Care Needs
For people with intermediate care needs who need regular and ongoing support with daily living.
Level 4
High Level Care Needs
For people with high level care needs who require extensive support to remain living at home.
How to Apply
Contact My Aged Care
Call 1800 200 422 or visit myagedcare.gov.au to register and request an assessment.
Complete Assessment
An assessor will visit you to understand your needs and recommend a package level.
Choose Zolentra
Once approved, select Zolentra Support Services as your provider.
Start Your Care
We'll create a personalized care plan and begin providing support.
